The adult mentor for the team should be the one to fill this out.

For IN PERSON sessions: If teens are in view, please make sure they have a CN/AT&T Media Release. You may include images that show hands working/the back of the head if you are not sure of their media release status. Adults trained do not need a media release.

For LIVE ONLINE sessions: Please provide clear screenshots of the session that show people were in the live session. For example, we can see the participants in the main window.

For WEBINAR style trainings: Please include screenshots of the video you sent out.
