Teens Teach Tech Program Plan

Welcome to Teens Teach Tech!

Before starting: Please note this document asks for financial information and verification that you are the adult mentor for the team. ONLY the Adult Mentor should complete this form.

This document will outline the details about your organization and how you plan to conduct the training sessions for Teens Teach Tech. This should be filled out when you are sure that your organization will participate in the program and you have organized a team of teens to participate.

Due to the need for financial information, it is very important that you have already submitted/are in the process of submitting the necessary paperwork (Program Contract, W9 and tax information) to the Program Coordinator.

Please read through each question carefully. If you have any questions, please reach out to your Mentor Guide or Program Coordinator.

20. Is your team allowed to receive financial incentives for their participation in the program? *This question is required.

21. Would you like to apply for start-up funding? These funds are used to pay for any supplies you may need to put on the training sessions.

(You must have financial documents submitted to receive funds. For direct deposit, please request an EFT Authorization Form if you have not received one. For check payments, you will need a W-9 and Tax paperwork on file). Start-up funding approval begins June 2026. Start-up funds are issued up to 30 days after Program Plan submission is received. *This question is required.
